Gitsegukla Band is seeking a health professional to fill the role of Community Health Nurse for our organization. This is a professionally and personally rewarding opportunity for someone who wants to have a significant impact on the well-being and everyday lives of a First Nation community.
Under the direction of the Health Manager, the Community Health Nurse is responsible for the assessment and coordination of client care within the community health programs offered at Gitsegukla. The
Community Health Nurse works effectively within the framework of the population health/health promotion model, to help increase the provision of quality care in this unique setting.
Key Responsibilities
General duties include:
Program and Service Delivery Management:
- Identify needs and respond with appropriate health services.
- Coordinate care services to meet the needs of community members while encouraging independence, family and community support.
- Develop client-centered care plans.
- Refer community members to appropriate agencies and/or physicians.
- Provide clients and family caregivers with health information as required through demonstrations and informal teaching sessions.
- Provide professional support and consultation to ensure the programs are delivered in a safe effective manner.
- Promote mental health and wellness.
- Support the Health Manager to assess, plan, implement and evaluate community health programming.
Stakeholder Relations and Management
- Cultivate internal and external linkages and partnerships with other healthcare professionals and collaborate with community resources.
- Maintain a professional, open, equitable and service-oriented relationship with everyone
Skills and Qualifications
Knowledge, Skills and Abilities:
- Knowledge and understanding of the history of First Nation communities in Canada and the current health and social issues along with cultural awareness
- Excellent written and interpersonal communication skills and the ability to work effectively with a variety of people and circumstances
- Ability to teach, facilitate learning, and support adoption of health programs and practices
- Self-directed with demonstrated organizational skills
Experience and Education Requirements:
- Member in good standing of the College of Registered Nurses of British Columbia (CRNBC)
- Bachelor of Nursing degree from a Canadian University or equivalent combination of training and experience
- 1+ year experience working in a similar position in a rural / remote setting and with First Nation communities considered an asset
- Experience working with palliative or elders care considered an asset
- Willing to gain additional certification, education and skills as required
- BCCDC Immunization Certification (or willingness to obtain)
Ability to successfully and periodically undergo an appropriate criminal records review and police record check is required.
